Note: A local contact number is essential as we will need to call you upon arrival at your location. Without one, it may be difficult to coordinate effectively.
Base fee for the mobile service good for: (a) 10 puppies/kittens; or (b) 4 cats/small-sized dogs [1-10kg]; or (c) 2 medium-sized dogs [10.1-25kg]; or (d) 1 large-sized dog [>25kg].
Note: No same day pick-up. No 11:30 AM to 12:30 PM pick-up.

BOOKING AGREEMENT

The estimated total provided is an approximation and may be subject to adjustments based on specific circumstances. For pets exceeding the base weight requirement, the trip cost may increase, potentially doubling.

Booking and Scheduling
• Book your pet pick-up service at least 24-48 hours in advance.
• Notify us at least 12 hours before the scheduled pick-up time for cancellations or rescheduling. Late cancellations will result in forfeiture of payment.
• Provide accurate details regarding the pick-up address, contact information, and any special access instructions.

Conditions for Declining Trip Requests
We reserve the right to decline a trip request under the following conditions:
• The pet’s location is beyond the serviceable radius of our branches (e.g., more than 5 kilometers away).
• The pet’s location poses a risk to the safety of our vehicle or staff.
• The owner does not comply with the guidelines outlined in our Pet Mobile Form.
• The pet has severe, uncontrollable behavioral concerns or poses a safety risk.
• The request for pick-up is made on the same day as the request submission.

Preparation for Pick-Up
• Containment: Provide a leash, harness, or carrier/crate appropriate for your pet’s size and behavior.
• Pet Readiness: Ensure your pet is ready at the agreed pick-up time and location. Pets should be secured in a designated area if not handed over directly. Drivers or assistants are not responsible for retrieving pets from the client’s premises, nor securing leashes or carriers.
• Cleanliness: Ensure your pet is clean and free of fleas, ticks, or other parasites. Pets with parasites may be treated at an additional cost.
• Diapers: All pets must wear a diaper during transport. If the owner does not provide a diaper, one will be supplied by the company at an additional cost.
• Necessary Items: Pack essential items such as food, toys, blankets, or medication, clearly labeled with your pet’s name.

Agreement Terms
By pressing SUBMIT, you agree to:
• Pay the fees associated with the transportation and servicing of your pet at any of our clinics.
• Not hold Aycardo Veterinary Center Inc. (AVCI) liable for any unforeseen or unfavorable circumstances during the transportation and servicing of your pet.
• Acknowledge that the company is not responsible for pre-existing health conditions, injuries caused by the pet’s behavior, or items improperly secured by the owner.

Emergency Situations
In the event of an emergency during transport, we will contact the owner and provide necessary veterinary assistance. Any costs incurred will be billed to the owner.

Once the form is submitted, expect a call from our staff within 24 hours (excluding Sundays) to confirm the details.
